PARTICIPATION AGREEMENT, ASSUMPTION OF RISK, RELEASE OF LIABILITY,
MEDICAL AUTHORIZATION, AND PHOTO/MEDIA RELEASE

I, the undersigned, hereby make application for myself and/or my minor child(ren) to participate in training, classes, events, and activities offered by Yesha Ministries, Inc., including but not limited to Yesha Taekwondo classes, training sessions, introductory lessons, sample classes, pilot lessons, tryouts and trials, demonstrations, exhibitions, kickathons, test events, competitions, tournaments, seminars, sparring, board breaking, physical conditioning, and other martial arts-related activities.
Upon acceptance into the program, I sincerely pledge that I and/or my minor child(ren) will obey all rules, regulations, instructions, safety requirements, and codes of conduct established by Yesha Ministries, Inc., Yesha Taekwondo, and its instructors. I understand that these rules are established for the purpose of maintaining order, promoting discipline, and helping protect participants from injury. I further understand that participation in martial arts requires attentiveness, respect for instructors, self-control, and adherence to instructor direction at all times.
1. Assumption of Risk
I understand and acknowledge that participation in Taekwondo, martial arts, physical training, sparring, breaking, demonstrations, tournaments, and related activities involves physical exertion and carries inherent risks of injury. These risks may include, but are not limited to, bruises, sprains, strains, fractures, concussions, serious bodily injury, permanent disability, property damage, or death.
I voluntarily assume full responsibility for all risks of bodily injury, death, or property damage that may occur as a result of my and/or my minor child's participation in any Yesha Ministries, Inc. or Yesha Taekwondo activity, whether such injury or damage is caused by the negligence of the Releasees named below or otherwise.
2. Release of Liability and Covenant Not to Sue
In consideration of being permitted to participate in Yesha Ministries, Inc. and/or Yesha Taekwondo activities, I, for myself, my minor child(ren), and our personal representatives, assigns, heirs, and next of kin, hereby release, waive, discharge, and covenant not to sue Yesha Ministries, Inc., its directors, officers, employees, agents, members, instructors, volunteers, authorized guest instructors, representatives, and affiliates, hereafter referred to as the "Releasees," from any and all liability, claims, demands, actions, causes of action, losses, damages, injuries, or death arising out of or related to participation in Yesha Ministries, Inc. or Yesha Taekwondo activities.
This release applies to any loss, damage, injury, or death sustained by the participant(s), or any loss or damage to property belonging to the participant(s), whether caused by the negligence of the Releasees or otherwise, while participating in, observing, traveling to or from, or otherwise being present for Yesha Ministries, Inc. or Yesha Taekwondo activities.
3. Indemnification and Hold Harmless Agreement
I agree to indemnify, defend, save, and hold harmless the Releasees from any and all loss, liability, damage, cost, expense, claim, judgment, or demand, including attorney's fees where applicable, arising out of or related to my and/or my minor child's participation in Yesha Ministries, Inc. or Yesha Taekwondo activities. This indemnification applies whether such claims arise from accidents, injuries, property damage, negligence of the Releasees, or otherwise.
4. Facility Release
I also release, waive, discharge, and covenant not to sue any church, school, nonprofit organization, community center, recreational facility, property owner, property operator, municipality, or other organization whose premises or facilities are used for Yesha Ministries, Inc. or Yesha Taekwondo activities. This release includes all liability, claims, judgments, or demands arising from accidents, injuries, death, or property damage occurring while the participant is in, upon, or about the premises or facilities where Yesha Ministries, Inc. or Yesha Taekwondo activities are taking place, whether caused by the negligence of the facility owner/operator or otherwise.
5. Medical Authorization and Responsibility for Medical Costs
I understand that I am responsible for determining whether I and/or my minor child(ren) are physically able to participate in Taekwondo, martial arts, and related physical activities.
In the event of injury, illness, or medical emergency, I authorize Yesha Ministries, Inc., Yesha Taekwondo instructors, staff, volunteers, or authorized representatives to seek emergency medical treatment for the registered participant(s) if I cannot be reached or if immediate care appears necessary.
I understand and agree that I am fully responsible for all medical needs and all costs associated with any emergency medical care, treatment, transportation, or related services.
I further understand that membership in or insurance coverage through any outside Taekwondo, martial arts, or athletic association, including the USA Tae Kwon Do Association where applicable, may require separate application, separate fees, and separate acceptance. Any such coverage is not guaranteed by this agreement.
6. Parental Consent for Minor Participants
If signing on behalf of a minor participant, I affirm that I am the parent or legal guardian of the minor participant(s) listed in this registration and that I have full authority to execute this agreement on their behalf. I understand that by signing this agreement, I am agreeing to all terms on behalf of myself and my minor child(ren), including the assumption of risk, release of liability, indemnification, medical authorization, code of conduct, and photo/media release.
7. Code of Conduct and Program Rules
I agree that all participant(s) listed in this agreement will abide by the rules, expectations, and code of conduct established by Yesha Ministries, Inc., Yesha Taekwondo, its instructors, and authorized representatives. Participants are expected to show respect toward instructors, volunteers, fellow students, families, facilities, and equipment. Participants must follow safety instructions, use self-control, and refrain from behavior that is unsafe, disruptive, disrespectful, or contrary to the standards of the program.
I understand that failure to comply with program rules, instructor direction, or the code of conduct may result in suspension or removal from the program without refund.
8. Photo and Media Release
I hereby grant Yesha Ministries, Inc., Yesha Taekwondo, and their authorized representatives permission to photograph, video record, livestream, or otherwise capture images, audio, video, or other media of the participant(s) during classes, training sessions, tournaments, demonstrations, ceremonies, sanctioned events, or related activities. I authorize Yesha Ministries, Inc. and Yesha Taekwondo to use such photographs, videos, recordings, or media for promotional, educational, ministry, informational, social media, website, advertising, or marketing purposes, without expectation of compensation.
9. Term of Agreement
This release, waiver of liability, assumption of risk, indemnification agreement, medical authorization, code of conduct agreement, and photo/media consent shall remain in full force and effect for a period of exactly one (1) year from the date of signature below. This agreement applies to all classes, training sessions, introductory lessons, sample classes, pilot lessons, tryouts and trials, demonstrations, exhibitions, kickathons, test events, competitions, tournaments, seminars, ceremonies, and events attended by the registered participant(s) during this one-year period.
10. Severability and Governing Law
I expressly agree that this Participation Agreement, Assumption of Risk, Release of Liability, Medical Authorization, and Photo/Media Release is intended to be as broad and inclusive as permitted by the laws of the State of Florida. If any portion of this agreement is held invalid or unenforceable, I agree that the remaining portions shall continue in full legal force and effect.
11. Acknowledgment of Understanding
I acknowledge that I have carefully read this agreement, understand its contents, and voluntarily sign it. I understand that this is a legally binding document and that by signing it, I am giving up certain legal rights, including the right to sue the Releasees for claims covered by this agreement.
I further agree that no oral representations, statements, promises, or inducements apart from this written agreement have been made.
